Expression of Wish Form - Nominate a Beneficiary

You can use this form to nominate who you would like to receive the one off lump sum death grant.

Please note that the form must be signed by hand, we are not able to accept digital signatures due to the sensitive nature of the nomination. 

You can submit the completed form to the Fund by post, or you can scan or photograph both sides of the form and email it to pensionenquiries@essex.gov.uk.
