Change of Name Form

Form used to notify the Fund of a change of name.

Please note that this form must be signed by hand as we are unable to accept digital signatures for security reasons. The form must be accompanied by a copy of the official document which shows your change of name, for example your marriage or civil partnership certificate or deed of name change.

You can return the form and a copy of the supporting document by post, or you can scan or photograph the form and documents and email them to pensionenquiries@essex.gov.uk.
